At Clover Health, we are revolutionizing healthcare with our groundbreaking primary care platform, Counterpart Assistant. Our mission is to empower Primary Care Physicians (PCPs) to enhance patient care while reducing costs through early diagnosis and comprehensive management of chronic conditions.
We are in search of a Provider Success Manager who will build strong relationships with healthcare providers, promoting the adoption and success of Counterpart Assistant (CA). This role involves managing a portfolio of provider partnerships and ensuring effective implementation and utilization of our platform to excel in value-based care.
This results-oriented role is focused on customer success, serving as an external product subject matter expert (SME). By leveraging your knowledge of Medicare and Primary Care operations, along with exceptional relationship management skills, you will contribute to expanding our market presence and enhancing the value we deliver to our partners. Candidates must be located in Houston and should be prepared to travel to practices up to five days a week.
As a Provider Success Manager, you will:
- Own & Manage Provider Relationships: Act as the primary contact for provider primary care partners to foster trusted relationships and drive adoption of CA.
- Support Provider Recruitment Activities as Product SME: Collaborate with the CA go-to-market (GTM) team to implement effective onboarding and training practices, focusing on optimizing in-office workflows and closely working with both clinical and non-clinical staff to ensure successful platform adoption.
- Optimize Provider Success & Retention: Develop and manage a structured engagement plan to guarantee that providers fully utilize our platform, achieve measurable success in value-based care, and maintain long-term partnerships.
- Lead Data-Driven Decision-Making: Analyze provider performance metrics, proactively identify trends, opportunities, and risks with partner practices, and lead initiatives to improve provider outcomes and increase platform utilization.
- Influence and drive adoption of best practices: Implement CA best practices across diverse PCP practices.
- Cross-Functional Player: Work collaboratively with internal teams, including sales, product, and operations, to enhance Counterpart Health’s offerings based on provider feedback and market trends.
