Under the guidance of our Medical Director, you will collaborate closely with WelbeHealth Physicians and the IDT to perform assessments, plan patient care, and educate our participants and their families.

Your Daily Responsibilities Will Include:

  • Providing continuous medical coverage and conducting comprehensive in-person assessments for our participants, including initial, semi-annual, and unscheduled evaluations.
  • Working with Physicians and other IDT members to create care plans, ensuring effective coordination, and managing discharge planning from various settings.
  • Maintaining participant care in skilled nursing facilities and residences through regular visits as needed.
  • Analyzing outcomes and utilization data to establish best practices and enhance policy improvements.
