What You'll Need:Board Certification with a valid NP or PA certification in Virginia. Demonstrated clinical skills with the capability to accurately assess, diagnose, and treat patients. Positive attitude and excellent interpersonal skills. Ability to independently problem-solve and make decisions confidently.1-2 years of experience in primary care, particularly with geriatric patients (hospice experience preferred but not mandatory).

Role Overview

This Nurse Practitioner or Physician Assistant position focuses on delivering primary care to homebound patients in Richmond, Virginia. The role involves working under physician supervision and managing a patient population with chronic illnesses.

Key Responsibilities

  • Provide primary and palliative care to patients in the Richmond area, including home visits.
  • Complete health assessments through physical exams and detailed medical histories to create care plans.
  • Communicate with office staff and clinical teams using internal communication tools.
  • Deliver patient-centered care to assigned patients.
  • Develop clinical impressions, diagnoses, and treatment plans based on evaluations.
  • Build trust and engage with patients, families, and caregivers during visits.
  • Order diagnostic tests as needed.
  • Prescribe medications and interventions when medically appropriate.
