Role Overview
This Nurse Practitioner or Physician Assistant position focuses on delivering primary care to homebound patients in Richmond, Virginia. The role involves working under physician supervision and managing a patient population with chronic illnesses.
Key Responsibilities
- Provide primary and palliative care to patients in the Richmond area, including home visits.
- Complete health assessments through physical exams and detailed medical histories to create care plans.
- Communicate with office staff and clinical teams using internal communication tools.
- Deliver patient-centered care to assigned patients.
- Develop clinical impressions, diagnoses, and treatment plans based on evaluations.
- Build trust and engage with patients, families, and caregivers during visits.
- Order diagnostic tests as needed.
- Prescribe medications and interventions when medically appropriate.
