Qualifications

Qualifications: Board Certified with valid NP or PA certification in New York State. Demonstrated strong clinical skills and the ability to accurately assess, diagnose, and treat patients. Exhibit a positive attitude and possess excellent interpersonal skills. Proficient in problem-solving and able to work independently.1-2 years of experience in primary care, preferably with geriatric patients; hospice experience is highly preferred but not mandatory.

Purpose: 

We are seeking a skilled Advanced Provider (Nurse Practitioner or Physician Assistant) to deliver exceptional primary and palliative care to homebound patients in the Prospect Heights and Flatbush neighborhoods of Brooklyn, NY. This role involves applying medical techniques under the guidance of a physician while catering to a diverse patient population with chronic illnesses.

Your Responsibilities: 

  • Deliver high-quality primary and palliative care to patients in Brooklyn, NY.
  • Conduct thorough health assessments through physical examinations and by obtaining and reviewing medical histories to formulate optimal care plans.
  • Maintain effective communication with office staff and clinical operations across various internal platforms.
  • Provide patient-centered care to a designated panel of patients.
  • Participate in quarterly medical staff meetings.
  • Develop impressions, diagnoses, and treatment plans.
  • Build trust and consult with patients, families, and caregivers during visits.
  • Order necessary diagnostic tests as required.
  • Prescribe medications, supplies, and other medically necessary interventions.
