Position Overview:As the Licensed Clinical Lead, you will serve as the primary clinical steward for your designated cases, ensuring that all aspects of the care plan are executed with adherence to the Positive Development (PD) model. This role emphasizes fieldwork, involving direct clinical observation, supervision, and coaching of Developmental Paraprofessionals (DPPs) and their families. You will work collaboratively with caregivers, interdisciplinary team members, and operational partners to achieve measurable outcomes while upholding the highest standards of clinical quality and compliance.Clinical Supervision and OversightConduct in-home and community visits to provide hands-on clinical supervision, observing DPP sessions, and delivering real-time feedback.Ensure that DPPs implement interventions in alignment with the Positive Development model and the clinical treatment plan.Perform fidelity checks and document performance metrics to guide ongoing training and development of DPPs.Collaborate with DPP Managers to assess field performance, identify support needs, and uphold model integrity.Ownership of Cases and Treatment Strategy DevelopmentExecute thorough clinical assessments (in-person) to assess developmental, behavioral, and relational functioning.Facilitate meetings with parents/caregivers to set family goals, priorities, and strategies for collaboration.Create, implement, and maintain personalized treatment plans that align with PD’s model and payer guidelines.Track client progress through data analysis and observation, adjusting goals and interventions as required.Lead interdisciplinary case reviews with internal and external providers (e.g., OT, SLP, MH, CM).Engagement with Families and CommunityFacilitate regular family coaching and educational sessions—both virtual and in-person—to enhance caregiver capacity and engagement.Organize community-based sessions to generalize skills across various environments.Assist families in navigating care transitions and accessing community resources as necessary.Documentation and Compliance...
