Job Summary
We are in search of a dedicated and compassionate Cardiology Nurse Practitioner or Physician Assistant to join our dynamic team. In this pivotal role, you will provide comprehensive care to patients facing heart-related conditions. This is a unique opportunity to positively impact patient lives while collaborating with a passionate team of healthcare professionals.
Responsibilities
- Conduct thorough patient assessments, including physical examinations, medical histories, and diagnostic tests.
- Work in collaboration with cardiologists to formulate and execute treatment plans for patients with cardiovascular issues.
- Administer medications, monitor patient responses, and adjust treatment strategies as needed.
- Perform cardiac stress tests, electrocardiograms (EKGs), and other diagnostic procedures.
- Educate patients and their families regarding cardiovascular health, preventive practices, and the importance of medication adherence.
- Provide emotional support and counseling to patients and their families.
- Familiarity with CMS Regulations and NCQA HEDIS Guidelines.
- Maintain accurate medical records in compliance with OSHA and government regulations, performing duties in line with site quality and safety standards.
- Collaborate with an interdisciplinary team to enhance patient care.
- Analyze and organize care for critically complex patients.
Requirements
- Minimum of 6 months of experience in a long-term care or skilled nursing facility.
