Salary Range: $75,000 to $84,000 per year
Position Overview
Join our growing Integrated Behavioral Health (IBH) team in Oakland, CA, as an Associate Mental Health Therapist. In this crucial role, you will deliver compassionate, person-centered support across behavioral health, housing, and social service systems. Key Responsibilities
Care Coordination & Program Support
- Provide both in-person and telehealth-based care coordination for clients with behavioral health and social support needs.
- Conduct thorough client assessments and assist in developing personalized care plans.
- Facilitate seamless handoffs and referrals to behavioral health, medical, housing, and community resources.
- Monitor client engagement and support ongoing care to ensure service continuity.
- Collaborate with internal care teams and external partners to meet client needs holistically.
- Participate in multidisciplinary team meetings, case reviews, and service planning sessions.
- Accurately document services and client progress in the electronic health record (EHR).
- Assist with data collection and service tracking to fulfill reporting requirements.
Compliance & Training
- Ensure adherence to Medi-Cal, HIPAA, and internal documentation protocols.
- Attend mandatory training sessions, community forums, and IBH meetings.
- Support program evaluation activities by monitoring outcomes and service delivery.
- Contribute to outreach, resource navigation, and sustainability initiatives.
Qualifications
- Registered Associate Clinical Social Worker (ASW/ACSW) in California.
- Bachelor’s or Master’s degree in Social Work, Psychology, Public Health, or a related field.
- A minimum of 1 year of relevant experience in behavioral health, case management, or social services.
- Familiarity with Alameda County care systems, including Medi-Cal and community resources.
- Understanding of trauma-informed and culturally responsive service delivery.
- Excellent communication, time management, and interpersonal skills.
- Able to work collaboratively with both clinical and non-clinical team members.
Preferred Qualifications
- Experience in integrated or primary care settings.
- Proficiency in an electronic health record system (SmartCare or similar).
- Bilingual skills (Spanish or other languages) are highly valued.
- Reliable transportation and a valid California driver’s license.
Work Environment
This position is office-based in Oakland with a supportive and collaborative team atmosphere.
