Join CareHarmony as a Licensed Practical Nurse (LPN) where you will collaborate with healthcare providers to implement value-based care management for our patients.

We are looking for a dedicated and skilled LPN with a minimum of 3 years of hands-on patient care experience. You should thrive in a dynamic environment, possess self-motivation, exhibit exceptional attention to detail, and appreciate the significant role you play in enhancing patients’ healthcare journeys.

Your role will involve recognizing resources and coordinating care for patients managing chronic conditions.

Key Responsibilities:

  • Oversee patient census with a focus on resolving clinical and non-clinical care gaps.
  • Identify and facilitate community resources that enhance patient care.
  • Educate patients on managing chronic conditions and improving health literacy.
  • Conduct medication management, including identifying issues, reconciliation, adherence, and refill coordination.
  • Ensure timely delivery of essential services, such as Home Health, DME, and Home Infusion.
  • Address patient inquiries and foster open communication to understand their needs.
  • Assist with referrals and appointment scheduling.

Essential Qualifications:

  • Current New York LPN License
  • Proficiency with Microsoft Office Suite
  • Strong written and verbal communication skills

Preferred Qualifications:

  • Experience with Epic Software
  • Bilingual abilities
  • Additional LPN state licenses

Remote Work Requirements:

  • Reliable high-speed internet connection
  • Designated home office or HIPAA-compliant workspace

Physical Requirements:

  • This is a sedentary role requiring prolonged periods of sitting.
  • Clear verbal communication and attentive listening are essential, often conducted over the phone.
  • The ability to understand and document real-time notes from phone conversations is necessary.
