Qualifications
Key Responsibilities:Manage patient census with a focus on resolving clinical and non-clinical care gaps. Identify and connect patients with community resources that enhance their health outcomes. Deliver patient education on chronic condition management and promote health literacy. Oversee medication management, including identifying potential issues, conducting reconciliations, ensuring adherence, and coordinating refills. Facilitate timely service delivery for Home Health, DME, Home Infusion, and other urgent patient needs. Address patient inquiries and foster open communication to better understand their needs. Assist in managing referrals and scheduling appointments. Additional Qualifications:Must have an active New York LPN license. Proficient in Microsoft Office Suite. Strong verbal and written communication skills. Preferred Qualifications:Experience with Epic software. Bilingual candidates are encouraged to apply. Additional state LPN licenses are a plus. Remote Work Requirements:Active high-speed Wi-Fi connection. A dedicated home office or HIPAA-compliant workspace. Physical Requirements:This position involves sedentary work, requiring extended periods of sitting. Ability to communicate clearly and listen attentively for long durations, primarily via telephone. Capability to comprehend, process, and take detailed notes during real-time phone conversations.
