The Registered Nurse Case Manager at Luminis Health plays a key part in supporting the care management team in Annapolis, MD. This weekend-only position involves 8-hour shifts and works closely with the clinical director to help patients move safely and efficiently through different stages of care. The role combines direct patient interaction with administrative work, such as documentation and insurance review, to ensure each patient receives timely access to the right resources.

What you will do

  • Assess and prioritize patients for care management by considering medical, functional, psychosocial, cultural, spiritual, and financial factors. Work with a multidisciplinary team to create comprehensive care plans.
  • Use engagement strategies, including motivational interviewing, to support patient self-care, encourage adherence to treatment plans, and promote overall well-being.
  • Apply evidence-based guidelines, such as InterQual, to standardize care and reduce unnecessary variation. Complete clinical reviews and authorizations in Epic, and refer cases to the Physician Advisor when appropriate.
  • Manage observation stay patients by coordinating timely tests, treatments, and transitions to either inpatient care or discharge.
  • Create and coordinate transition plans for patients returning home or moving to post-acute care, such as home health, hospice, or palliative services. Ensure all documentation is thorough and handoffs to the next providers are clear.
  • Maintain detailed records that capture patients' physical and functional limitations, psychosocial background, and educational needs for both patients and families.
