Job Overview
We are seeking a dedicated Primary Care Nurse Practitioner / Physician Assistant (NP/PA) to join our team and provide on-site care to residents in long-term care settings. This full-time role is perfect for a provider who values meaningful patient connections and thrives in a collaborative care environment.
Key Responsibilities
- Deliver primary care services to residents in skilled nursing and long-term care facilities
- Conduct physical examinations and medical history assessments
- Diagnose and manage chronic and acute health conditions
- Order and interpret diagnostic tests and lab results
- Create and modify personalized care plans
- Prescribe medications and evaluate treatment effectiveness
- Collaborate with facility staff and interdisciplinary teams
- Educate patients and their families about treatment options and care goals
- Maintain a safe, respectful, and supportive environment for residents
Qualifications
- Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license
- State-specific license in good standing in New York
- At least 6 months of experience in long-term care or a skilled nursing facility
- Strong communication, clinical, and critical thinking abilities
- A genuine passion for elder care and person-centered healthcare
- Ability to lift a minimum of 20 pounds and stand/walk/work on your feet for up to 6-8 hours per day
