Minimum 6 months of experience in long-term care or a skilled nursing facility. Strong communication, clinical, and critical thinking abilities. A genuine passion for elder care and person-centered healthcare. Ability to lift at least 20 pounds and stand/walk/work on your feet for 6-8 hours a day.

Job Summary

We are actively seeking a Primary Care Nurse Practitioner / Physician Assistant to deliver on-site care to residents in long-term care environments. This part-time role is perfect for a healthcare professional who prioritizes meaningful patient relationships and thrives in a collaborative care setting.

Responsibilities

  • Deliver primary care services to residents in skilled nursing and long-term care facilities.
  • Conduct physical examinations and medical history assessments.
  • Diagnose and manage chronic and acute health conditions.
  • Order and interpret diagnostic tests and laboratory results.
  • Develop and modify individualized care plans.
  • Prescribe medications and assess treatment efficacy.
  • Work in cooperation with facility staff and interdisciplinary teams.
  • Provide education to patients and families regarding treatment options and care objectives.
  • Maintain a safe, respectful, and supportive atmosphere for residents.
