Luminis Health seeks a part-time Case Manager (RN) for weekends in Annapolis, MD. This position focuses on coordinating patient care across settings, working closely with the Clinical Director of Care Management. The Case Manager assesses both health and social factors that affect patient outcomes and ensures patients receive timely, appropriate resources and support. Responsibilities include using evidence-based guidelines to determine care levels and handling denial reviews for Medicare, Medicaid, and commercial payers.

Key Responsibilities

  • Identify and prioritize patients who need care management, considering biopsychosocial, functional, cultural, spiritual, and financial factors. Collaborate with a multidisciplinary team for assessment and planning.
  • Engage patients using motivational interviewing and other strategies to encourage self-care, treatment adherence, and improved health outcomes.
  • Apply evidence-based guidelines (such as InterQual) to support quality care, reduce variability, and minimize unnecessary services. Verify care levels, enter clinical reviews, and maintain documentation to prevent denied days.
  • Manage observation stay patients assertively, ensuring timely testing and treatment, and facilitate either conversion to inpatient status or safe discharge.
  • Develop and coordinate transition plans for patients moving to home care, community care coordination, hospice, palliative care, or skilled post-acute providers. Complete all required documentation and handovers.
  • Keep thorough, clear documentation in patient records, including details on physical and functional limitations, psychosocial characteristics, educational needs, and available support systems for patients and families.

Schedule and Location

This is a part-time position based in Annapolis, MD. Weekend availability is required.
