Purpose:

We are seeking a skilled Nurse Practitioner or Physician Assistant to provide exceptional primary and palliative care to homebound patients in Mercer County, NJ. You will work under the supervision of a physician, applying your medical expertise to cater to a diverse population of patients with chronic illnesses.

What You'll Do:

  • Deliver high-quality care to primary and palliative patients in and around Mercer County, NJ.
  • Evaluate patient health through physical exams, review of medical histories, and formulate optimal care plans.
  • Communicate effectively with office staff and clinical operations through various internal platforms.
  • Provide patient-centered care to a designated panel of patients.
  • Participate in quarterly medical staff meetings.
  • Formulate diagnoses and treatment plans based on patient assessments.
  • Build trust and consult with patients, families, and caregivers during home visits.
  • Order necessary diagnostic tests and prescribe appropriate medications and interventions.
