Your Role

As a Behavioral Health Care Manager, you will serve as the primary liaison for patients enrolled with April Health. Your responsibilities will include coordinating patient care with primary care clinicians and consulting psychiatrists, utilizing validated measurement tools to track patient symptoms, and directly delivering evidence-based treatments.

The Collaborative Care Model adopts a population-centric strategy for managing mental health within primary care, deviating from traditional psychotherapy approaches. We focus on brief, evidence-based interventions, such as motivational interviewing, problem-solving treatment, and behavioral activation.

Key Responsibilities:

  • Collaborate closely with the patient's primary care provider to develop a mutually agreed-upon care plan for mental health treatment.
  • Conduct screenings and assessments for common mental health disorders.
  • Assist in the management of psychotropic medications prescribed by primary care providers, emphasizing adherence, side effects, and treatment efficacy.
  • Monitor patient follow-up and clinical outcomes using a registry, documenting encounters, and employing the system to reconnect with patients as needed.
  • Establish specific self-management goals and actionable behavioral interventions with motivated patients.
  • Facilitate adjustments to care plans for patients who are not progressing, including medication or psychosocial treatment changes and referrals for additional services.
  • Engage in regular consultations with psychiatric consultants regarding your caseload.
