The Opportunity
Join us as a Substance Use Navigator, where you will be the guiding hand for individuals seeking assistance, transitioning them from “I need help” to “meds in hand.” In this role, you will conduct Scope AI-powered intakes, collaborate with prescribers for prompt medication reviews, navigate pharmacy challenges, and ensure seamless patient transitions to Community Health Workers (CHWs) for ongoing management.
What You’ll Do
- Intake & triage: Conduct motivational interviewing-informed intake assessments using Akido’s Scope AI powered Addiction Treatment Intake, alongside SBIRT and ASAM assessments. Identify safety concerns and make warm transfers to crisis services as necessary.
- Consent & privacy: Secure and document patient consent, adhering to redisclosure regulations and utilizing approved scripts/forms.
- Navigate care quickly: Verify Medi-Cal eligibility and secure a pharmacy capable of dispensing medications like buprenorphine or naltrexone.
- Pharmacy follow-through: Proactively communicate with pharmacies to resolve prior authorization and stock issues, confirm medication pickup or delivery, and keep prescribers and case managers informed.
- Follow-ups: Work with case managers to perform structured check-ins to monitor patient progress and adherence.
