As a Social Work Assistant, you will play a crucial role within the IDT, directly impacting the lives of our participants. Join us in helping them reach their full potential!
The Social Work Assistant is responsible for managing a caseload of participants while working closely with the IDT and Social Worker. This role serves as a vital link between participants, their families, and the IDT by offering guidance, executing treatment plans, coordinating community services, and handling essential documentation.
Key Responsibilities:
- Build relationships with participants and their families to provide emotional and psychosocial support.
- Implement and document social work interventions within the agreed scope of practice, including arranging necessary resources and facilitating transitions in care settings.
- Collaborate with IDT members to coordinate services related to care, such as housing assistance, DMV needs, utility support, and other community resources.
- Conduct IDT care plan reviews with participants and family members within required timelines.
- Perform home visits, hospital visits, and skilled nursing visits as necessary.
- Assist in the completion of grievances, appeals, incident reports, APS reports, and other compliance-related documents.
