Program Overview
The Advanced Care Partner Program creates a collaborative team-care structure that works alongside Primary Care Providers (PCPs) managing complex patient panels.
This model pairs an Advanced Care Partner (ACP) with a PCP, allowing for shared responsibility and continuity of care for the patient panel. The ACP is dedicated to maintaining all service level agreements (SLAs), responding to patient messages in real-time, and closely monitoring chronic care, ensuring that patients receive the highest quality of care and support. This approach promotes sustainability in careers, reduces the risk of decision fatigue, and mitigates burnout, prioritizing the well-being of both patients and providers.
Advanced Care Partner Role:
The ACP is a full-time, remote clinician who collaborates with PCP peers and clinical supervisors during business hours. They will manage approximately five PCPs and their patient panels, maintaining accountability over time and ensuring seamless continuity of care.
