As a dedicated Healthguide, you will play a crucial role in enhancing the relationship between patients and their primary care physicians, fostering improved health and well-being. Your responsibilities will include guiding patients through their healthcare journey, addressing their concerns, and facilitating communication with medical practice staff.
Key Responsibilities:
- Work primarily from home, collaborating with the patient’s medical practice team and Guidehealth’s Case Management team to manage a defined group of patients.
- Operate independently while being a proactive team member, extending the capabilities of the medical practice.
- Support patients in achieving optimal health outcomes by answering their inquiries, connecting them with their medical practice, and assisting in the implementation of impactful care plans while addressing any care gaps.
- Document patient medical histories and issues accurately within the electronic health record (EHR).
- Comprehend the referral and prior authorization processes.
- Develop and maintain a professional and trustworthy relationship with assigned medical practices and the Guidehealth organization.
- Meet assigned Key Performance Indicators to ensure positive health outcomes while managing medical expenditures.
- Utilize various communication methods (calls, texts, portals, emails) to engage and schedule high-risk patients.
- Escalate medical issues appropriately to the Health Guide Registered Nurse, Primary Care Physician, or Guidehealth leadership.
- Adhere to all security policies and procedures to safeguard Protected Health Information (PHI) and Guidehealth’s intellectual properties.
- Willingness to travel for patient and provider engagement and for conducting retinal eye exams in the Atlanta, GA area, with compensation offered for travel (10-15%).
