Your Role
We are seeking a dedicated Registered Nurse to fill the position of RN Case Manager, reporting directly to the Director of Nursing in Colorado. This Full-Time Hybrid role is based in the Colorado Springs area.
As an RN Case Manager at Abby Care, you will oversee the holistic care and coordination of services for your patients, demonstrating exceptional organizational skills, strong communication, and a commitment to delivering high-quality care. You will collaborate closely with patients, families, Primary Care Physicians (PCPs), therapy providers, and internal support teams to ensure the best possible patient outcomes.
Key Responsibilities:
Patient Care and Coordination:
- Manage a diverse caseload, ensuring quality patient care and effective coordination of services.
- Provide guidance, oversight, and support to Home Health Aides (HHAs) to uphold high standards of patient care.
- Develop and implement personalized care plans based on thorough patient assessments and established goals.
- Coordinate with PCPs, therapy providers, and internal support teams to ensure comprehensive patient care.
- Educate and support patients and their families on healthcare needs and treatment plans.
Clinical Documentation:
- Maintain accurate and timely electronic health records, emphasizing real-time documentation.
- Ensure all documentation adheres to regulatory standards and organizational protocols.
