The Population Health Associate plays a crucial consulting role in advancing population health initiatives for our patient population. This role collaborates closely with the Quality, Risk Adjustment, and Practice Operations teams to formulate and execute effective action planning and goal-setting with designated providers and care centers, concentrating on enhancing quality, coding, and documentation to bolster performance within value-based care agreements. Responsibilities include both virtual and in-person meetings with Privia providers to address focal areas.
Key Responsibilities:
- Analyze performance data from payers and Privia analytics to generate actionable reports for providers, outlining specific measurable performance goals aligned with broader team or organizational objectives regarding quality, coding, and documentation.
- Engage in discussions regarding data and actionable steps with providers and care center staff during regular one-on-one meetings, fostering accountability for success within designated care centers.
- Maintain ownership and accountability over the performance metrics of assigned care centers and providers within the specified geography.
- Establish and cultivate collaborative relationships with assigned care centers to enhance performance in Value-Based Care Programs.
- Deliver training on quality measures and other presentations as required for care center staff and providers.
- Leverage expertise in EMR and prior medical office experience to offer tailored workflow guidance, best practices, and troubleshooting to enhance performance in value-based care.
- Examine medical records to pinpoint opportunities for closing quality measure gaps and tracking trends.
- Submit supplementary data to the relevant payer portals.
- Participate in special projects and undertake additional responsibilities as assigned.
