Position: Physical Therapist (PT) - Home Health
Employment Status: Full-Time, Part-Time, or Per Diem
Location: You will have a caseload tailored to your preferred location.
(OPO Health is licensed in over 15 counties.)
Key Responsibilities:
Patient Care:
- Provide physical therapy services according to the physician’s care plan, including:
- Evaluating patients’ therapeutic, rehabilitative, and functional status.
- Collaborating in the development and adjustment of the care plan.
- Assessing muscle strength, mobility, gait, range of motion, and rehabilitation potential.
- Administering and directing physical therapy treatments.
- Instructing patients and caregivers on the use and maintenance of therapeutic equipment.
- Identifying and prioritizing physical therapy needs.
- Reporting treatment responses and changes in patient conditions to the physician.
- Initiate therapy programs and guide family members and caregivers in specific therapy phases to facilitate patient progress.
- Educate patients and caregivers on therapy objectives and methods, engaging in case conferences as needed.
- Recognize the need for additional home health services and make appropriate referrals.
- Prepare and submit clinical documentation and progress reports based on patient achievements.
- Contribute to discharge planning for smooth transitions of care.
Communication:
- Maintain accurate clinical notes and provide updates to the primary physician as necessary.
- Effectively communicate with physicians regarding patients’ conditions, needs, and required adjustments to care plans.
