The Pharmacy Liaison plays a pivotal role in providing comprehensive support to patients and pharmacists, ensuring a hands-on approach to outstanding patient care. This involves working closely with patients to facilitate medication adherence through our innovative tools and applications, while encouraging specialty clinic patients to take full advantage of our pharmacy's exceptional care services.
Located within a medical center clinic, seamless integration with the clinic team—including physicians, nurses, and support staff—is vital for success in this role.
Key Responsibilities:
- Deliver exemplary care to hospital patients through a complete range of tailored medication management services.
- Collaborate effectively with hospital clinicians as a member of the clinic team, providing assistance to patients both in person and over the phone.
- Educate prospective pharmacy patients about the hospital’s services and enhance their overall care experience.
- Address outpatient retail and specialty prescription medication needs.
- Expand the specialty pharmacy’s patient population.
- Monitor patients’ progress with medication regimens through follow-up therapy and medication adherence check-ins; secure refill prescriptions as necessary.
- Maintain effective communication with the centralized Patient Service Center regarding refill reports, zero-fill reports, monthly outstanding reports, and appointment lists.
- Contribute to the development of new processes to enhance operational efficiency and patient care.
- Identify medications that require special storage or handling, including hazardous or refrigerated medications.
- Determine necessary supplies for specific medications, including sharp objects.
