Key Responsibilities:
- Implement care plans tailored to patients' unique needs.
- Conduct home visits to clients across designated areas.
- Administer medications, perform wound care, and execute treatments as required.
- Regularly monitor and report on clients' health status to Clinical or Client Services Manager.
- Follow up on and accurately document physician orders.
- Conduct client assessments as necessary.
- Engage in case management and care coordination.
- Accurate documentation of observations and care activities.
