Position Overview:
As a Full-Time Level 2 Registered Nurse Case Manager, you will work primarily in the field, visiting patients in their homes within the Martinez/Pleasant Hill Area. In this leadership role, you will oversee a team of two nurses, guiding them in implementing comprehensive care plans aimed at promoting wellness and preventing re-hospitalization.
Your responsibilities will include conducting thorough patient assessments, including Start of Care evaluations, Resumption of Care, and discharge planning. You will also provide essential education and assistance to patients and their families regarding wound care, blood draws, tube feeding, tracheostomy care, foley catheter management, CHF management, and medication education.
