The Home Care Coordinator is an essential member of our team, responsible for conducting in-home care evaluations and establishing the framework for our home care team to enable participants to flourish. Reporting directly to the Home Care Manager, this role centers on organizing, assessing, and supervising personal care within participants' residences.

Your Daily Responsibilities Will Include:
- Managing incoming communications related to participants, physicians, and agency services concerning physician orders, participant inquiries, and referrals.
- Engaging with participants via phone and coordinating effectively with nursing, therapy, aides, social services, and physicians regarding schedule modifications, test results, and other pertinent updates.
- Collaborating with Home Care Services staff to track and monitor the delivery of home care and scheduling hours.
- Partnering with the Marketing team to assist in enrolling prospective participants into the program.
- Supporting staffing and scheduling activities, soliciting and managing input from various departments.
- Participating in ongoing training and development opportunities to enhance skills and knowledge.
