This position involves delivering patient care at the patient's home, following a physician-established care plan. As a 1099 contract opportunity, this role allows for the development of long-lasting relationships with patients, as our services do not expire once a patient is accepted into the program.
The patient in question requires low-acuity care, specifically 2 hours of assistance per week, preferably on Monday mid to late morning.
You will have the opportunity to advocate for the patient and collaborate with a team that includes CNAs, HHAs, case managers, and physicians to ensure comprehensive care.
