- Assist patients with personal care activities as outlined in their care plan, including hygiene, mobility, and elimination; report any inability to perform tasks to the RN or Director of Clinical Services.
- Ensure patient safety by maintaining a secure environment and using equipment correctly.
- Implement Universal Infection Control Precautions diligently in all patient interactions.
- Help patients and families with home maintenance tasks as defined in the care plan, including light housekeeping and meal preparation.
- Monitor and record patients' fluid and nutrition intake, as well as vital signs such as temperature, pulse, and respiration, and report any changes appropriately.
- Utilize safe transfer and positioning techniques to assist patients in mobility and ambulation.
- Identify and communicate the need for intervention from other home health team members to the RN or Director of Clinical Services.
- Respect the confidentiality and autonomy of all patients and their families.
- Document care provided and patient responses accurately and submit documentation promptly.
- Operate within the bounds of your training and experience.
- Adhere to local, state, and federal regulations, as well as Medicare standards and agency policies.
- Manage your caseload effectively while providing care in a cost-efficient manner.
- Consistently meet or exceed the service standards established by 365 Health Services.
- Conduct all activities in a professional and ethical manner.
- Utilize supplies and equipment efficiently.
- Complete at least 12 hours of in-service training annually.
- Maintain consistent attendance.
- Follow the 365 Health Services Code of Conduct.
- Interact positively with all staff, promoting the mission and vision of 365 Health Services.
- Comply with agency policies, including confidentiality and HIPAA regulations.
