As a Clinical Pharmacist, you will be instrumental in promoting medication adherence and elevating quality outcomes for Medicare patients and those involved in value-based care programs.

You will collaborate with healthcare providers, Healthguides, and care teams to bridge adherence gaps, boost HEDIS performance, and assist patients in achieving optimal health outcomes.
Please note: An active, unrestricted pharmacist license in Georgia (GA) is required for this position. Candidates lacking an active GA license will not be considered.
Key Responsibilities
HEDIS & Quality Improvement
- Lead initiatives aimed at enhancing HEDIS medication adherence and pharmacy quality measures, including adherence to antihypertensive, cholesterol-lowering, and diabetes medications.
- Analyze adherence and pharmacy data to pinpoint care gaps and create targeted, scalable interventions.
- Collaborate with providers, Healthguides, care managers, and internal teams to enhance quality performance within a value-based care framework.
Patient Engagement & Clinical Interventions
- Facilitate Medication Therapy Management (MTM) sessions to identify barriers to adherence and optimize medication regimens.
- Educate patients empathetically about medication usage, potential side effects, and dosing schedules.
- Utilize motivational interviewing techniques to promote behavior change and sustained adherence.
- Tackle real-world challenges such as medication costs, side effects, and regimen complexity.
Data Analysis & Reporting
- Monitor and evaluate HEDIS performance metrics to assess the effectiveness of adherence programs.
- Leverage EHR and pharmacy data to proactively identify patients at risk for non-adherence and formulate actionable care plans.
- Prepare and present insights to leadership, translating data into recommendations for continuous improvement.
Collaboration, Training & Subject Matter Expertise
- Act as a subject matter expert (SME) in medication adherence across Guidehealth.
- Support and educate pharmacists, Healthguides, care managers, and clinical partners on adherence best practices.
- Engage in cross-functional initiatives that focus on quality improvement, population health, and scalable value-based care solutions.
