As a Clinical Pharmacist, you will play a vital role in identifying, preventing, and resolving medication-related issues. You will provide authoritative information to healthcare professionals, patients, and the public, ensuring that pharmacy services operate as an integrated system focused on patient-centered care for all patients and staff at Northwestern Medicine.

Key Responsibilities:
- Interpret medication orders to guarantee safety and proper medication use, while being vigilant about compatibilities, incompatibilities, medication duplication, allergies, and potential administration errors. Facilitate problem-solving between pharmacy, nursing, and prescribers, demonstrating initiative and sound judgment.
- Monitor medication therapy for significant drug-drug, drug-diet, or drug-laboratory test interactions and intervene in cases of significant interactions or therapeutic duplications.
- Assess and document the appropriateness of selected medications for desired therapeutic outcomes, communicating potential issues and solutions to prescribers.
- Serve as a consultant on pharmacokinetics and adverse reactions to physicians, nurses, and other healthcare professionals. Participate in therapeutic drug monitoring or intervention programs, adjusting therapy based on patient conditions and laboratory results.
- Process medication order information into the Electronic Health Record to ensure accuracy in records regarding dispensing and charging.
- Maintain comprehensive pharmacy patient profiles, incorporating age, weight, allergies, diagnoses, and pertinent information affecting medication therapy decisions.
- Oversee packaging, labeling, and distribution of medications for patients and employees, adhering to established policies and procedures.
