Role Overview
We are looking for dynamic and dedicated Clinical Care Leads to join our innovative care team, reporting to the Clinical Operations Manager. This role demands robust clinical knowledge, adaptability, and the capability to excel in a fast-paced, patient-focused environment. The Clinical Care Lead will play a vital role in chronic care management (CCM) and advanced primary care models (APCM) by directly engaging with patients, supporting care plans, and conducting clinical triage. About the Position
The ideal candidate is a compassionate, organized, and technologically adept Clinical Care Lead, focused on assisting patients with chronic conditions through our text-based Advanced Primary Care Management (APCM) program. This remote role emphasizes delivering proactive, high-quality care via digital communication tools, ultimately driving improved health outcomes for Medicare and older adult populations.
This position is perfect for healthcare professionals who thrive in a virtual care setting, prioritize patient engagement, and possess strong critical thinking and care coordination skills.
Key Responsibilities
- Act as the primary point of contact for patients through a secure text-based platform
- Timely and professionally monitor and respond to patient communications and check-ins
- Assess and triage clinical concerns, social determinants of health (SDOH) needs, and care coordination challenges
- Create and maintain personalized care plans for patients with chronic illnesses (e.g., hypertension, diabetes, COPD)
- Design, implement, and refine care coordination workflows or protocols to enhance efficiency and scalability of care models
