We are thrilled to announce an opportunity for a passionate Care Coordinator to become a vital member of our expanding Continuing Healthcare team in South Yorkshire. This dedicated team delivers exceptional care and support to individuals with complex healthcare needs, allowing them to thrive in their own homes and in residential care settings, particularly during end-of-life care.
As a Care Coordinator, you will work within a close-knit, multidisciplinary team, collaborating closely with the Regional Manager to ensure the highest standards of care. Your responsibilities will include:
- Creating and managing weekly staff rotas, ensuring accurate updates in our internal rostering system.
- Communicating with care workers daily to facilitate smooth operations and address any emerging issues.
- Engaging with local authorities and liaising with families and representatives of private clients.
- Participating in local authority review meetings to uphold care standards.
- Conducting client risk assessments, reviews, and spot checks to ensure safety and quality of care.
- Maintaining and updating client behavioral and care charts for effective monitoring.
- Assisting in the onboarding process for new care workers.
- Processing weekly staff payroll efficiently.
- Completing compliance paperwork for local authorities.
- Performing general administrative duties as needed.
- Participating in an evening on-call rotation, typically once per week.
- Providing care shift coverage in urgent situations.
While previous experience in care coordination is preferred, it is not essential. Candidates must possess a full driver’s license, have access to a vehicle, and be willing to travel throughout South Yorkshire.
This is a full-time position, requiring 37.5 hours per week, Monday to Friday.
