Position Overview
At Gotham Enterprises, we are on the lookout for a dedicated Associate Clinical Social Worker (ASW/ACSW) to become an integral part of our Integrated Behavioral Health (IBH) team in Berkeley, CA. This role is perfect for early-career clinicians eager to enhance their skills in integrated care while receiving comprehensive clinical supervision. You will serve as a vital link, connecting clients with crucial behavioral, medical, and social services.
Key Responsibilities
Care Coordination & Program Support
- Provide in-person and telehealth care coordination for individuals requiring behavioral health and social support.
- Conduct thorough client assessments and contribute to personalized care planning.
- Facilitate warm handoffs and referrals to various resources including behavioral health, medical, housing, and community services.
- Track client engagement and support follow-up care to ensure continuity of services.
- Work collaboratively with internal care teams and external partners to address client needs holistically.
- Engage in multidisciplinary team meetings, case reviews, and service planning sessions.
- Maintain accurate documentation of services and client progress in the electronic health record (EHR).
- Assist in data collection and service tracking to fulfill reporting requirements.
Compliance & Training
- Ensure compliance with Medi-Cal, HIPAA, and internal documentation standards.
- Participate in mandatory training sessions, community forums, and IBH meetings.
- Support program evaluation efforts by monitoring outcomes and service delivery.
- Contribute to outreach initiatives, resource navigation, and sustainability efforts.
Qualifications
- Registered Associate Clinical Social Worker (ASW/ACSW) in California.
- Bachelor’s or Master’s degree in Social Work, Psychology, Public Health, or a related discipline.
- At least 1 year of relevant experience in behavioral health, case management, or social services.
- Familiarity with Alameda County care systems, including Medi-Cal and community resources.
- Knowledge of trauma-informed and culturally responsive service delivery.
- Excellent communication, time management, and interpersonal skills.
- A collaborative spirit to work effectively with both clinical and non-clinical team members.
Preferred Qualifications:
- Experience in integrated or primary care settings.
- Proficiency in electronic health record systems (e.g., SmartCare).
- Bilingual skills (Spanish or other languages) are highly desirable.
- Reliable transportation and a valid California driver’s license.
