Inspired by the successful model of Brilliant Corners in California, we facilitate a seamless apartment search, approval, and move-in experience while bridging the gaps within the NYC voucher housing system. Our mission is to be a transformative force for New Yorkers facing homelessness or at risk of losing their homes.
Position Overview:
We are actively seeking a skilled social worker to join our team as an Aftercare Specialist - Rapid Rehousing. In this crucial role, you will report directly to the Aftercare Supervisor - Rapid Rehousing and be responsible for guiding Participants throughout their program enrollment to achieve and maintain housing stability before, during, and after their placement. This position involves conducting comprehensive needs assessments for all Participants and collaborating with households to address their objectives related to education, employment, health, and mental wellness.
As the primary contact for Participants from referral through housing and aftercare, you will also work closely with the Participant’s support network, which includes the Aftercare Supervisor - RRH, the Aftercare Associate – Opportunity Navigator, and Property Providers when necessary.
Key Responsibilities:
- Evaluate the immediate needs of clients and make suitable referrals to facilitate rapid rehousing and support their stabilization in permanent housing.
- Identify the strengths and barriers faced by Participants, aiding them in overcoming obstacles and connecting them with essential resources and services.
- Deliver aftercare, case management, and advocacy services tailored to the needs of Participants.
- Conduct proactive follow-up home visits to ensure stability and foster progress towards self-sufficiency, which includes providing support, advocacy, reducing isolation, problem-solving, and identifying resources.
- Maintain biweekly phone contact and quarterly in-person visits once Participants are housed to monitor overall safety and wellness.
- Offer referrals to community resources for assistance related to mental health, wellness, legal matters, and other support areas as needed.
- Participate in meetings and case conferences to ensure collaborative support is effectively provided to Participants for their stability.
- Respond to emergencies during business hours and after hours, being part of the on-call roster for all Anthos|Home Participants and Property Providers.
