This is a field-based role necessitating regular travel to patients' homes in Brooklyn and Queens, New York.

Position Overview:

The Health Home Care Manager will engage with potentially eligible patients, securing appropriate consents for their enrollment in the Health Home Care Management Program. Post-enrollment, the Manager will collaborate with the patient's care team to carry out a thorough assessment and develop a patient-centered care plan while coordinating care in alignment with that plan.
