Exceptional communication and interpersonal skills.

BAYADA Home Health Care is seeking a passionate and dedicated Habilitation Manager (Qualified Professional) for our State College, PA office. This is a fantastic opportunity to advance your career in the growing field of home health care, where you can make a real difference in the lives of our clients.

Key Responsibilities:

  • Serve as an internal case manager and advocate for clients within the Habilitation program, collaborating closely with the Clinical Manager.
  • Engage in Person-Centered Planning processes to tailor care to individual client needs.
  • Work in tandem with clients, families, Case Managers, Habilitation Technicians, and other professionals to assess and implement care plans based on client goals.
  • Conduct regular client visits to monitor progress and ensure services align with the Person-Centered/Individual Support Plan.
  • Evaluate client goal progress and maintain communication with Case Managers and Care Coordinators.
  • Supervise Habilitation Technicians while adhering to policy and state regulations.
  • Participate in client-specific orientations as instructed by the Clinical Manager.
