Positive demeanor and excellent interpersonal skills. Ability to problem-solve and work independently.1-2 years of experience in primary care, particularly with geriatric patients; hospice experience preferred but not mandatory.

Purpose: We are seeking an experienced Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to deliver primary and palliative care to homebound patients in the Brooklyn/Queens area. You will utilize your medical expertise under physician supervision to care for a diverse patient population experiencing chronic diseases.

What You'll Do:

  • Deliver high-quality care to primary and palliative patients in the Brooklyn/Queens area.
  • Perform physical examinations, assess health, and develop personalized care plans.
  • Communicate effectively with office staff and clinical operations across various platforms.
  • Provide patient-centered care to your assigned panel of patients.
  • Participate in quarterly medical staff meetings.
  • Formulate impressions, diagnoses, and treatment plans.
  • Build trust with patients, families, and caregivers during visits.
  • Order necessary diagnostic tests as required.
  • Prescribe medications and interventions that are medically necessary.
