Position Overview:
We are expanding our team to enhance the quality care we provide to our members. We are currently looking for a highly skilled and empathetic certified Nurse Practitioner to join our Advantmed provider network. This role is pivotal in conducting in-home wellness risk adjustment assessments for Medicare members and other at-risk populations. Your primary focus will be to evaluate the overall health of member beneficiaries, ensuring accurate risk adjustment coding, which is crucial for achieving greater value-based care. This is a 1099 (PRN) position that offers significant flexibility, ideal for those managing multiple commitments.

Key Responsibilities:
- Conduct in-home clinical assessments, including travel to members’ residences for comprehensive health evaluations.
- Perform Annual Wellness Visits and health assessments for members with chronic conditions, which include:
- Taking medical history and conducting physical examinations.
- Reviewing systems and capturing vital signs.
- Medication review and reconciliation.
- Performing preventive health screenings such as Hemoglobin A1C, Spirometry, Kidney Health Evaluation (KED), Diabetic Retinal Exam (DRE), and FIT.
- Provide patient education and engagement through preventive health education and counseling opportunities.
