As a Field Nurse Case Manager, you will serve as a vital liaison in coordinating care and sharing critical medical information between the Injured Worker, the Insured, and the Carrier, ensuring optimal treatment, effective rehabilitation, and a smooth transition back to work. Your role will emphasize the delivery of timely, cost-efficient communication related to work injuries, motor vehicle accidents, liability claims, and other specialized services. This position allows for remote work with regular local travel of up to 2 hours within your designated region.

Key Responsibilities

  • Adhere to all policies and procedures outlined in the Case Management (CM) plan, maintaining accountability throughout the CM process.

  • Receive and manage referrals as assigned by the Director of Case Management.

  • Initiate contacts according to the CM plan, schedule initial assessments, and confirm the next physician appointment date.

  • Verify assignments with the referral source and clarify any special handling instructions.

  • Develop and nurture professional relationships with clients, treating all claimants with respect and dignity.

  • Maintain consistent communication with the Injured Worker, Insured, and Carrier following physician appointments.

  • Document data and billing accurately in CaseAnyplace and submit monthly reports in a timely manner (Preliminary, Initial, Progress, Closing).

  • Attend physician appointments to gather diagnosis, prognosis, treatment plans, rehabilitation timelines, and return-to-work (RTW) estimates.

  • Collaborate with Vocational Counselors for job analyses when necessary and approved by the Carrier.

  • Recommend and coordinate Independent Medical Examinations (IMEs) and attend these appointments.

  • Arrange transportation for clients as required.

  • Provide translation services when necessary (for bilingual nurses).

  • Monitor treatment plans and attend therapy sessions as appropriate, maintaining regular contact with therapists for updates.

  • Request file transfers to Vocational Counselors when necessary (LMS, Voc Rehab).

  • Assist the Carrier/Insured with return-to-work planning (modified or regular duty).

  • Provide relevant information to Defense Attorneys as required.

  • Encourage teamwork and collaboration among staff members.

  • Aim to maximize accurate billable hours, targeting 8 hours per day.
