Job Summary
As a Field Nurse Case Manager, you will act as a vital liaison responsible for coordinating care and conveying essential medical information between injured workers, insurers, and carriers. Your role is crucial in ensuring timely treatment, effective rehabilitation, and a smooth return to work. This position is predominantly remote and requires regular travel within your local area, typically not exceeding 2 hours from your base.
Duties & Responsibilities
Adhere to all policies and procedures outlined in the Case Management plan, ensuring accountability throughout the process.
Receive and manage referrals as directed by the Director of Case Management.
Initiate contact as per the Case Management plan, schedule assessments, and track upcoming physician appointments.
Confirm assignments with referral sources, clarifying any specific instructions.
Build and maintain professional relationships with clients, treating claimants with the utmost respect.
Follow up with carriers and insured parties after physician appointments, ensuring continuous communication with all relevant stakeholders.
Document all data and billing accurately in CaseAnyplace and submit monthly reports in a timely manner.
Attend physician appointments to gather crucial information regarding diagnosis, treatment plans, and recovery timelines.
Collaborate with Vocational Counselors for job analysis when necessary.
Recommend independent medical exam (IME) physicians and coordinate attendance at IMEs.
Arrange transportation for clients when needed.
Provide translation services if applicable.
Monitor treatment plans and maintain contact with therapists for updates.
Assist with file transfers to Vocational Counselors as required.
Support return-to-work planning for clients.
Provide relevant information to defense attorneys as necessary.
Foster teamwork among all staff members.
Maximize billable hours to meet monthly targets of 8 hours per day.
Maintain necessary licensure and certifications.
