If you are seeking a fulfilling career in a dynamic and expanding industry, this position may be the perfect fit for you!As a Program Specialist, you will:Collaborate with the Clinical Manager to serve as an internal case manager and client advocate within the Habilitation program.Engage in Person-Centered Planning processes.Work with clients, families, Case Managers, Habilitation Technicians, and other teams to implement and assess client care needs and individual goals outlined in the Person-Centered/Individual Support Plan.Conduct regular client visits to monitor progress and ensure services align with the Person-Centered/Individual Support Plan and service definitions.Assess the achievement of goals established in the Person-Centered/Individual Support Plan.Maintain consistent communication with Case Managers and Care Coordinators.Supervise Habilitation Technicians in accordance with policies and state regulations.Participate in client-specific orientations as directed by the Clinical Manager.
