Candidates must reside within a 20-mile radius of the assigned territory due to travel frequency.This is a critical role that we aim to fill promptly.Key ResponsibilitiesAs a Lead Care Manager, you will perform hybrid (in-field and remote) care management duties, including:Evaluate member needs across various domains, including physical health, mental health, substance use disorders, oral health, palliative care, memory care, trauma-informed care, social support, housing, and referrals to community services.Supervise the creation and implementation of client care plans and goal-setting.Provide services at locations most convenient for the member, whether through office visits, telehealth, or field services.Connect clients with additional social services and support as needed.
