The City and County of San Francisco is seeking a dedicated Pharmacist to join the Department of Public Health (DPH) under general supervision. In this pivotal role, you will ensure optimal patient care, manage appropriate drug therapies, and adhere to all relevant regulations.
Available positions span across various DPH locations, including:
- Zuckerberg San Francisco General Hospital & Trauma Center (ZSFG) Inpatient Pharmacy
- Zuckerberg San Francisco General Hospital & Trauma Center (ZSFG) Outpatient Pharmacy
- Zuckerberg San Francisco General Hospital & Trauma Center (ZSFG) Purchasing Pharmacy
- Zuckerberg San Francisco General Hospital & Trauma Center (ZSFG) 340B Program Coordinator
- San Francisco County Jails, including locations in San Francisco and San Bruno
- Community Behavioral Health Services
- Laguna Honda Hospital and Rehabilitation Center
- Various clinics and Health Centers throughout the City and County of San Francisco.
Some roles may involve direct interaction with individuals in secure facilities, the homeless population, those with substance use disorders, or individuals with potential infectious diseases. Flexibility for evening, night, weekend, and holiday shifts may be required, depending on the location.
As a Pharmacist, under the general guidance of the Supervising Pharmacist, your key responsibilities will include:
- Processing, compounding, preparing, labeling, and dispensing medication orders and prescriptions.
- Monitoring drug therapy for suitability, contraindications, interactions, adverse effects, and therapeutic/toxic dosing.
- Communicating relevant drug information and providing medication counseling to patients and healthcare staff.
- Conducting quality assurance through drug use evaluations and medication area inspections.
- Maintaining accurate documentation of patient care activities and adhering to regulatory standards.
- Supervising support personnel as required.
Moreover, Pharmacist roles in the designated areas will also include:
ZSFG INPATIENT PHARMACY POSITIONS: (These positions necessitate Special Condition #1 as outlined in the Minimum Qualification section of this announcement.)
- Dispensing physician medication orders and prescriptions, including IV admixtures and chemotherapy.
- Monitoring and following up on unresolved medication order issues, lab orders, and properly documenting clinical interventions in the EHR system according to provided guidelines.
- Offering therapeutic drug monitoring consultations.
