The Registered Nurse Case Manager plays a pivotal role within the interdisciplinary team (IDT), directly influencing participant care. Reporting to clinical leadership, the RN Case Manager is responsible for delivering and coordinating all aspects of care. This includes conducting essential PACE assessments, creating and adhering to individualized care plans, and facilitating smooth transitions in care.

Your daily responsibilities will include:
- Perform clinic nursing assessments to identify participant needs, preferences, and goals, actively engaging in IDT meetings to formulate care plans.
- Implement and document nursing interventions as outlined in care plans, ensuring a safe environment, promptly addressing physician orders, and administering medications accurately.
- Work collaboratively with the WelbeHealth PACE Physician and fellow IDT members to coordinate participant needs, communicating effectively with participants and their families to adjust care plans as necessary.
- Educate participants, their families, and caregivers on disease processes, self-care strategies, and preventive measures.
