Oncology Clinical Pharmacy Specialist

We are seeking a dedicated Ambulatory Oncology Clinical Pharmacy Specialist who will play a crucial role in enhancing patient care at a prestigious Academic Medical Center. This dynamic, patient-focused position involves developing meaningful relationships with patients and healthcare providers while delivering exceptional medication management.
In this role, you will work on-site at the clinic, collaborating closely with care teams and practicing under a Collaborative Practice Agreement (CPA). Your responsibilities will include conducting in-person and virtual patient assessments, ordering and monitoring evidence-based treatment plans, optimizing therapy, and addressing medication-related issues effectively.
Location: Onsite in Orange, CA
Salary: The salary range for this position in California is between $150,000 and $190,000 (Hourly Equivalent: Approximately $72.12 - $91.35 per hour, based on a 40-hour workweek), plus bonuses pursuant to any applicable bonus plan. The actual salary will be determined based on experience, geographic location, and other legally permissible factors.

Your Responsibilities:
- Deliver comprehensive medication management, including reconciliation, dose optimization, adherence support, and drug-level assessments in alignment with evidence-based standards.
- Collaborate with physicians, specialists, and interdisciplinary teams to create individualized care plans under a CPA, ensuring seamless medication coordination.
- Continuously monitor therapy effectiveness and safety, identifying opportunities for optimization and intervening as necessary.
- Support quality improvement initiatives and Risk Evaluation and Mitigation Strategies (REMS).
- Engage in patient education and counseling to promote medication adherence and understanding.
