Our multidisciplinary team, comprised of nursing staff, MDs, LCSWs, and Community Health Workers (CHWs), collaborates to address the social determinants affecting health. This includes vital aspects such as food security, housing accessibility, transportation, and financial resources. The Care Neighborhood Community Health Worker will conduct outreach to high-risk patients and families in the community, engage with them, and relay essential information back to the healthcare team to create personalized care plans. This role requires a deep connection with the community, allowing the CHW to act as a bridge between health services and the clinic, ensuring patients receive comprehensive support.

Your role will involve addressing member escalations, overseeing handyperson transitions, and resolving intricate service and scheduling challenges. This position demands a blend of empathy, sound judgment, and effective cross-team collaboration.You will serve as a trusted advocate for our members who require additional support, while also working closely with our Operations and Handypeople teams to guarantee exceptional member experiences.3-Month OutcomesContribute significantly to reducing member churn and enhancing satisfaction by managing complex member situations effectively.Timely response to incoming tickets, particularly focusing on Tier 2 cases and escalations, including service disruptions and high-risk churn scenarios.Act as a composed and confident representative for Honey Homes during sensitive conversations with members, fostering trust through clear communication and empathy.Manage handyperson assignments for designated members, ensuring proactive outreach, clear expectation settings, and coordination with Operations for a seamless experience.Collaborate with Handypeople and Operations via Slack and other tools for issue triage, visit preparation, and on-the-ground challenge resolution.Assist with visit pre-scoping, complex scheduling requirements, and membership inquiries that necessitate extensive product or policy knowledge.Identify members at risk of churn and proactively engage them with solutions, education, or adjustments to their plans to maximize their membership value.Meet performance metrics including SLA adherence, churn reduction, and successful handyperson transitions.Approach ambiguous member issues with a problem-solving mindset, working independently while recognizing when to engage cross-functional partners for resolution.
