Our multidisciplinary team, comprised of nursing staff, MDs, LCSWs, and Community Health Workers (CHWs), collaborates to address the social determinants affecting health. This includes vital aspects such as food security, housing accessibility, transportation, and financial resources. The Care Neighborhood Community Health Worker will conduct outreach to high-risk patients and families in the community, engage with them, and relay essential information back to the healthcare team to create personalized care plans. This role requires a deep connection with the community, allowing the CHW to act as a bridge between health services and the clinic, ensuring patients receive comprehensive support.
