What you will do Deliver ongoing primary care, including comprehensive initial, semi-annual, and unscheduled assessments for participants. Collaborate with WelbeHealth Physicians and the interdisciplinary team to create and update individualized care plans. Educate participants and families about health conditions, treatment options, and self-care strategies. Coordinate care and discharge planning with other providers and specialists. Visit participants in skilled nursing facilities and at home, adjusting visit frequency based on clinical needs. Review outcomes and utilization data to help inform best practices and guide policy improvements.
