Job Summary
The Field Nurse Case Manager acts as a crucial liaison, ensuring seamless communication and coordination of care for injured workers. This role facilitates effective treatment, rehabilitation, and reintegration into the workplace by engaging with the Injured Worker, Insured, and Carrier. The position is primarily remote and requires local travel within a two-hour radius.
Duties & Responsibilities
- Adhere to all established policies and procedures outlined in the Case Management (CM) plan, maintaining accountability for the CM process.
- Accept and manage referrals as designated by the Director of Case Management.
- Initiate contact based on the CM plan, schedule assessments, and gather information on upcoming physician appointments.
- Confirm assignments with referral sources and address any special instructions.
- Establish and nurture professional relationships with clients, ensuring respectful treatment of claimants.
- Maintain communication with all parties post-physician appointments, including the Carrier and Insured.
- Document and manage data using CaseAnyplace, submitting monthly reports in a timely manner.
- Attend physician appointments to collect critical information regarding diagnosis, treatment plans, and recovery timelines.
- Refer cases to a Vocational Counselor for job analysis as necessary, with Carrier approval.
- Coordinate and attend Independent Medical Exams (IMEs) as required, recommending physicians as needed.
- Facilitate transportation for clients when necessary.
- Provide translation services if applicable (for bilingual nurses).
- Monitor treatment plans and attend therapy sessions as appropriate, keeping in touch with therapists for updates.
- Assist in the transfer of files to the Vocational Counselor when appropriate.
- Support the Carrier/Insured in planning for the return to work.
- Provide relevant information to Defense Attorneys as necessary.
- Encourage teamwork and collaboration among all staff members.
- Optimize billable hours to meet monthly targets.
- Maintain current licensure and certifications.
